Mine is set to 0.54 but I feel like I should update to 0.91 - what if anything might break?

Unknown, but I think basically everything should be ready to go on that. If your blog fails to build, post logs here and some of us can help.
The key difference that I have seen is that .91 needs to have an index.html. The other version would use list.html for the purpose.
I think all of the “stock” themes are fine for 0.91 so only a theme author needs to worry about this.
It’s possible that some plugins may be incompatible but doubtful.
